The VNA offers a full range of home nursing services including skilled treatments and procedures, medication management and patient education. Our highly skilled nurses coordinate the entire home care team, monitor the patient's progress and keep the physician and family fully involved and informed. The physicians and home care nurses work together to develop a plan of care to achieve each patient's individual objectives. Specialized programs and services are provided to patients recovering from a variety of conditions including surgery, accidents, strokes or other acute or chronic illnesses such as asthma, diabetes, cancer, congestive heart failure and hypertension.

Cardiopulmonary Services: The VNA Cardiopulmonary program provides specialized care to meet the unique needs of patients with cardiac and pulmonary conditions. The efforts of the program work to prevent complications, shorten the length of hospital stays, decrease hospital readmissions, and improve the overall quality of life for cardiopulmonary patients, with a focus on teaching the importance of medication management, safety, diet and exercise.
Diabetes Services: The VNA Diabetes program helps patients understand their disease, while providing medical assistance and support. VNA team members focus on providing one-on-one care, which allows each patient to learn at his or her own pace. Patients are given the tools needed to recognize signs and symptoms of complications and emergency situations sooner in order to prevent complications.
Alzheimer's/Dementia Services: The VNA Dementia Alzheimer's program is designed to help patients remain in their homes for as long as possible. The goals of the program are to increase quality of life, keep patients physically and cognitively active, increase patient and caregiver understanding of dementia through assessment and teaching, and ensure that patients and their families are referred to the appropriate VNA programs and other community resources, as necessary.
Mental Health Services: The VNA Mental Health program provides in-home assessment, care and counseling for adults, teenagers and children with both mental health and medical disorders. Psychiatric nurses complete mental and physical evaluations as well as provide counseling for the patient and family, crisis intervention and referrals to inpatient mental health centers. Mental health conditions can all be treated with medication, therapy, counseling and supportive activity like those offered through the VNA Mental Health program.
Oncology Services: The VNA Oncology program provides care to patients with cancer diagnoses. The oncology nurses promote the health of individuals through education, support, and symptom management that focuses on the physical, mental, and social well-being of oncology/cancer care patients in the community.
Orthopedic Services: The VNA Orthopedic program assists patients through assessment, education and hands-on in home care. Orthopedic nurses help patients to achieve their goals of independence in conjunction with physical and occupational therapists. Additional services, such as medical social work, home health aide and registered dietician may also be used to assist in meeting patient goals. The program focuses on preventing complications and falls through assessment and education and has succeeded in decreasing patient rehospitalization. HomMed health monitoring systems may also be used to aid in monitoring patients, especially those with a history of cardiopulmonary disease.
Wound Care: The VNA Wound Care program provides effective treatment and management of acute and chronic wounds through consistent and comprehensive individualized care. Certified Wound Ostomy Continence nurses (WOCN) are available to consult on complex cases. The program provides education on independence and self-care, ongoing assessment and early intervention to promote healing and prevent complications via appropriate and cost-effective resources.
Asthma Management: The VNA Asthma Management program equips patients and their families with the knowledge and tools necessary for the day-to-day management of asthma. Skilled nurses develop a personalized plan based on an assessment of the patient's history, lifestyle and environment. The program's team members assist patients in learning how to manage medication and devices correctly, educate patients and families about the disease, and work to identify and control triggers within a patient's environment.
Referrals to the Asthma Management program requires an order from a patient's primary healthcare provider.
VNA HomMed - Home Health Monitoring: VNA HomMed allows our Home Care team to monitor patients on a daily basis in between scheduled visits. Each day, the in-home monitor talks the patient through an assessment, which includes heart rate, blood pressure, weight, oxygen saturation and temperature. Additional devices can also be used to collect and transmit more in-depth data such as blood glucose levels via a glucometer.

We are the only home care agency between Omaha and Denver utilizing the BladderScanTM (bladder scanner), allowing us to prevent unnecessary catheterization and subsequent adverse consequences, such as urinary tract infections.
The bladder scanner enables our team to better evaluate incontinence, eliminate unnecessary catheterizations and discomfort, and preserve patient dignity.
