Initiating VNA Home Care

We work with you, your physician and family to offer the highest quality care you deserve – in the comfort of your own home. Our goal is to provide peace of mind, independence and quality of life.

There are four primary steps to take to receive VNA Home Care:

Receive a Physician Order

A referral for home care services can be made by anyone involved in the patient’s care including the patient, family members, or others. Typically, referrals are triggered by a change in the patient’s condition identified at a physician office visit or during a hospital stay. If someone other than the physician makes a referral, VNA will contact the patient’s physician to approve orders for home care.

Discuss the Options

Together, the patient, physician, hospital planner and family can determine the necessary services and home care agency to best meet the patient’s needs.

Develop a Personalized Plan of Care

A VNA clinician will visit the patient’s home, evaluate their needs and, in conjunction with the patient and family, develop an overall care strategy, which is submitted to the physician for approval.

Provide Your Home Care Visits

VNA will provide intermittent home care visits by the appropriate clinician according to the plan of care. Each care plan is coordinated and supervised by a registered nurse or therapist to assure that the patient’s health care needs are being met on a continuous basis.
