Home Care Patient Services: Your contribution to this fund helps VNA provide home health and personal care to patients with acute or chronic illnesses who are unable to afford routine health care. Home care services work to treat current illnesses, rehabilitate and prevent further illness or injury in the most cost effective and efficient manner. Home health aide services are provided to elderly, low income and disabled patients, as well as to high-risk populations with limited personal resources.
Hospice Patient Care: Contributions to this fund help to provide hospice services for the uninsured, as well as medications for pain management and symptom relief, medical equipment and supplies to promote quality of life, teen and adult volunteers, and bereavement services including grief support groups to care for families after the loss of their loved one.
Community Health Services: Your contribution to this fund serves the community at large to foster wellness and independence. A strong emphasis is placed on prevention and intervention. Community programs include:
- Nursing services to children and adults living in homeless shelters
- Dependent care/homemaker companion services
- Flu prevention
- Health Watch personal response system
- MD2 medication administration and reminder system
- Home delivered meals for disabled, homebound persons under 60 years of age
- Operation Frontline classes provide low-income individuals nutrition education and cooking skills
Services for Moms and Babies: Your contribution to this fund enhances the health and quality of life for women, children and families, especially those in vulnerable situations. Programs include:
- English and Spanish childbirth education classes
- Breastfeeding education and support
- Prenatal and postpartum in-home nursing visits
- School health services
- In-home asthma management
HomMed Health Monitoring Services: Your contribution to this fund provides consistent nursing care to those who cannot afford it, allowing chronically ill patients to remain in their own homes and avoid repeated hospitalization or placement in a full-time care facility. The use of a home monitoring device allows patients vital signs to be observed seven days a week, and can detect changes in their health status in a timely manner. This frequent monitoring means patients can maintain their health while keeping the dignity and peace of mind that comes with staying in their own home.
Staff and Volunteer Development: VNA staff members and volunteers are key components to meeting the needs of individuals and families looking for home care, hospice and community health services. Your contribution will help in the three R's – Recruitment, Retention and Recognition of VNA staff and volunteers. In addition, your support allows further staff development through seminars and workshops that would otherwise not be available. This assistance to staff and volunteers allows VNA to better serve its clients.
General Support Endowment Fund : Your contribution to the VNA General Support Endowment Fund allows the VNA flexibility in times of crisis to support the greatest need. Funds may go towards providing an elderly woman with physical therapy in her home so that she can remain independent, tending to the medical and emotional needs of men, women and children in area homeless shelters who are in desperate need of nursing services, or reaching young pregnant women to make sure they receive sufficient prenatal care and education. This Fund is a growing account where the principle is left intact and the earnings are used on an annual basis to fund the needs of VNA.
